Considerations and questions for Key Transition Points - Young Carers

Many young carers will have their needs met through a universal transition. However, in some cases where young carers are vulnerable, a more targeted or specialist approach will be required. 

Transition into Primary School

  • Use the Plymouth City Council enhanced transition framework to support the process.
  • Use you're meet the parent' events and home visits to raise awareness of young carers, identify them, and provide information about what a young carer is.
  • Work closely with pre-school settings, nurses and health professionals to ensure that key information is shared ( in accordance with GDPR)

Transition into Secondary school

  • Use the Plymouth City Council enhanced transition framework to support the process.
  • Work closely with feeder schools to ensure that key information is shared (in accordance with GDPR)
  • Refer parents to the considerations at transition document
  • Work with young carers to consider producing a person centred profile of their caring role (Word doc, 180 KB)
  • Secondary schools young carers' leads can raise awareness with new year 7 pupils about what a young carer is and how pupils can develop confidence to self-identify.
  • Consider having some young carers who attend the secondary school available to provide support with transition visits or meetings.
