Therapy Team - Livewell Southwest
The Livewell Southwest therapy team provides therapy for adults in a community, rather than clinical, setting. People who use this service will be seen at their home (or care setting) where therapy is considered to be more effective in those settings, or if it supports that person overcome mobility issues.

This service adopts a multi-disciplinary approach; that means that team is built up of lots of different professionals with different specialisms. As a result, the therapy service can take a holistic view of the best treatment for each individual - and not offer a "one size fits all" approach. The types of healthcare professionals working within this service include: occupational therapists; speech and language professionals; physios and therapy support workers.
The aim of this service is to help people to build, and maintain, the best quality of life and independence for them. That will look different for each person, but by providing timely and high-quality rehabilitation that helps meet their changing needs, this service seeks to unlock each individual's potential.
Livewell Southwest adopts a goal-orientated approach to rehabilitation that helps people to avoid being readmitted to hospital, and offers long-term support.
