The Plymouth Assisted Discharge Service, from the British Red Cross, supports patients in getting home after spending time in the hospital.

We can support you for up to 72 hours with various tasks, including

  • Shopping support
  • Prescription collection
  • Return visits to G.P or hospital appointments
  • Support settling in at home
  • Prescription collection
  • Signposting into other agencies

For some tasks, we can also provide support for a further four weeks through our community support team.
