Plymouth enhanced transition framework: Post 16

The 'Preparing for Adulthood Transition Guidance' should be used as an electronic tool to support educators, families, young people, and other agencies in planning for a successful next step. This guidance can be used with any young person who will benefit from additional planning.

This guidance is part of Plymouth's Local Offer. The Local Offer provides children and young people with special educational needs or disabilities (SEND), families, and professionals with information in one place, helping them to understand what services they can expect from a range of local agencies (including their entitlements). The Local Offer covers provision for children and young people from birth to 25 and includes information about education, health, and social care services.

In this document, the term setting is used to refer to any school, college, or training provider. The term learner is used to refer to a pupil at a school or a young person at a college or training provider.
